SERVICE DESCRIPTION:CaCoon is a statewide public health nurse home visiting program that provides community-based CAre COordinatiON for children with special health needs. Children served in the CaCoon program have higher rates of immunization, annual well-child visits and dental care visits; and make fewer visits to the emergency department. Since family members have a central role in the care of their child, all services are planned around the desires and concerns of the family.

INTAKE PROCEDURE:Call for information and to register or visit our website to fill out an online referral form.

ELIGIBILITY:Families with a child, ages 0-21, with a disability or a chronic health condition

SERVICE HOURS:Clinic hours: Monday-Friday 8am-11am and 1pm-4pm

DOCUMENTS:None required at intake

LANGUAGES:English, interpretation by arrangement
